AMFS MyHealth Personal Health Record (PHR)

AMFS MyHealth "Personal Health Record" is a web base online personal
health record which allows users to create, store, view and manage personal health
information in one secure central location. Users can print out a report for medical
office visits or emergencies or access information at any time online. PHR functions
include:
Medication record, including Rx and OTC therapies. Members have the option to view
prescription claims data loaded from a pharmacy benefits
management company and report medication usage.
Medical history, including family history, conditions, treatments, allergies, and
immunizations.
Test and measurement results for standard laboratory tests and other biometric readings.
When available, data will be loaded directly
from the laboratory. Users also have the option to self-report test and measurement
data.
Provider, pharmacy, hospital, insurance, and emergency contact information.
Printer-friendly health record reports for both the user and their doctor.
Ability to import data from shared medical information sources such as primary care
physicians, hospitals, clinics, etc.

AMFS MyHealth "Health Risk Assessment" (HRA)

AMFS MyHealth Health Risk Assessment is a web based tool that calculates
an individual’s health risks based on the responses to questions about current health
status, personal medical history, family medical history and lifestyle.
The HRA is an on-line questionnaire, is quick and easy to complete and provides
immediate feedback. The HRA report creates a personal baseline of health and wellness
that can be used to identify key opportunities for health improvement.
The on-line questionnaire includes branching logic that presents questions to the
user based on age, gender, and other data entered into the questionnaire.
Once a questionnaire is completed, the user has the option to lock the HRA, which
can provide permission-based access to health coaches, and/or health providers,
to the online reports.
Assessment and Reports will be generated from the questionnaire responses which
will assess, identify and quantify the users’ health risks in the following areas:

Both the personalized report and a medical provider summary report (for use at their
next medical office visit) are available online for the user to print and maintain.
The HRA reports are color coded to insure that the user can quickly focus in on
those health issues that are the most significant. HRA data is securely stored and
identified by user ID, to allow for inclusion in population health management programs.
AMFS MyHealth is unique, providing focused HRA’s available for Adult, College Age,
Teenager and Pediatric populations. The resultant findings on the HRA can be used
by the system to facilitate the formation of a more targeted AMFS MyHealth Action
Plan.

AMFS MyHealth Action Plans (MAPs) solves the problem of how to
attain one’s health and wellness goals by providing detailed plans for daily, weekly,
and/or monthly execution. MAPs gives users the documentation and step by step programs
that provide easy to use, interactive personal action plans that get you on the
road to better health.
MAPs are available in self-directed mode for personal health improvement, or with
a coach/instructor directed model. All action plans can be configured to match unique
client service levels and business rules including the duration of the program,
action steps to be included, supporting instructions and reporting requirements.

The AMFS MyHealth PHD offers a unique user interface that can be custom designed
to track the key health and wellness elements that make up one’s life. A dashboard
is a Visual Display of the most important information needed to achieve one or more
objectives; consolidated and arranged on a single screen so the information can
be monitored at a glance. With the busy lives that we all lead, one can now get
a snap short of what is important to you based on your health, life style, age,

An additional feature of the AMFS MyHealth PHD environment is the fact that each
of the visual components are “Web Widgets”. These widgets can be mixed and match

AMFS MyHealth "Personal Medical Images" (PMI)

AMFS MyHealth Personal Medical Imaging (PMI) is a module designed
to allow access from any location to scanned Electronic Medical Records. This feature
is a bridge product that makes medical records of all patient files securely available
to authorized users, at any time, simultaneously. The following is a list of the
suggested files that should be maintained in your AMFS MyHealth Personal Health
Record environment:
- Identification Sheet-A form originated at the time of registration
or admission. This form lists your name, address, telephone number, insurance, and
policy number.
- Problem List-A list of significant illnesses and operations you
have had.
- Medication Record-A list of medicines prescribed or given to you.
This form often lists any medication allergies you may have.
- History and Physical-A document that describes any major illnesses
and surgeries you have had, any significant family history of disease, your health
habits, and current medications. It also states what the physician found when he
examined you.
- Progress Notes-Notes made by the doctors, nurses, therapists, and
social workers caring for you that reflect your response to treatment, their observations
and plans for continued treatment.
- Consultation-An opinion about your condition made by a physician
other than your primary care physician. Sometimes a consultation is performed because
your physician would like the advice and counsel of another physician.
- Physician's Orders-Your physician's directions to other members
of the healthcare team regarding your medication's, tests, diets, and treatments.
- Imaging and X-ray Reports-Describe the findings of x-rays, mammograms,
ultrasounds, and scans. The actual films are maintained in the radiology or imaging
departments or on a computer.
- Lab Reports-Describe the results of tests conducted on body fluids.
Common examples include a throat culture, urinalysis, cholesterol level, and complete
blood count (CBC). Surprisingly, your health record does not usually contain your
blood type. Blood typing is not part of routine lab work.
- Immunization Record-A form documenting immunizations given for
disease such as polio, measles, mumps, rubella, and the flu. Parents should maintain
a copy of their children's immunization records with other important papers.
- Consent and Authorization Forms-Copies of consents for admission,
treatment, surgery, and release of information.
Each of these files can be scanned into you personal health repository and accessed
at any time from any location.
